Connecticut Dental Group, LLC, is dedicated to providing all our patients with a warm and hospitable atmosphere in which to receive top-quality dental care. We understand the apprehension that many people feel when going to the dentist, so we strive to make sure your experience with us is a pleasant one. Our staff is professional, friendly and fully qualified to meet all your needs. Our office was established 18 years ago, and Dr. Azar has over 10 years of experience.

Sterilization
Our office meets or exceeds all standardsof sterilization and infection control.
Appointments
Please try to be punctual for your scheduled appointment. We understand that emergencies arise that may prevent you from keeping a scheduled appointment. If you are aware of the need to miss your appointment please call the office 48 hours in advance. We have many patients waiting for appointments and we want to be able to service everyone to the best of our ability.

Insurance
We welcome most insurance plans. Dental insurances intended to cover some, but not all of the cost of your treatment. Most plans include patient co-pay or deductible, expenses which must be paid by the patient. If you have insurance please bring your information with you and we will be glad to submit the claim for you.
Payment Options
We make every effort to minimize the cost of your care. You can help by paying for treatment at the time of your visit. For your convenience we accept all major credit cards, as well as cash or check.
A 5% discount is available for payment in full of your personal balance at the time of treatment. (This does not apply to payment made by credit card.)
Payment of the balance in full on your statement should be made within 30 days of receipt unless other payment arrangements have been agreed to. All account balances outstanding more than 60 days from treatment date will be subject to a late charge fee of 18% annually.
We offer: Care Credit, The Capital One Healthcare Plan, and Citi Health Plan. They are made available through our office to help you in making easy low or no interest monthly payments.
